Proof of Identity and Address

As part of your request, we need to confirm your identity. This may require that you provide proof of identity or proof of address documentation. The following documentation may be requested. 
 
Proof of Address is not required with Human Trafficking Block requests. Human Trafficking Block requests also require victim determination documentation and a statement of specific items of information that you are requesting be removed as being the result of human trafficking.

Driver's License 
Must be valid, current, and in your name. The state of issuance must be clear and legible.
State or Government ID Card
Must be valid and current. Your name and state of issuance must be clear and legible.
Passport
Must be valid, current, and issued by the United States / US Territory.
W-2 or 1099 Form
Must be from the most recent tax year.
Paystubs
Must be dated within the past 60 days.
Social Security or Military ID Card
